Overview

The Social Support Coordinator will help address the support needs of those who are contacts, especially those who are under isolation or quarantine. This will include working with LHDs, community-based organizations, and support agencies to address housing, food, mental health, testing, medical, and other needs.

 

The position is temporary, full-time, and may end at any time depending on the demand.

Responsibilities

Social Support Coordinators shall be responsible for providing isolation and quarantine support and shall inform Workforce members about available resources and coordinate the referral of individuals diagnosed with COVID-19 and close contacts to the social, medical, and support services they need. Social Support Coordinators shall provide education about COVID-19, isolation, quarantine procedures, and relevant social support services to individuals diagnosed with COVID-19 and any close contacts. This support shall include working with local health departments, community-based organizations, and support agencies to address housing, food, mental health, testing, medical, and other needs. Specific duties of Social Support Coordinators are as follows:

 

Social Support Coordinators shall work collaboratively with Case Investigators from the Department and the Contractor and Contact Tracers to support the needs identified in communications with close contacts.

 

Social Support Coordinators shall maintain a list of available resources by county in coordination with the Department and other Commonwealth agencies.

 

Social Support Coordinators shall identify common needs amongst individuals diagnosed with COVID-19 and search for local solutions. Solutions could include, but are not limited to, food banks, mental health services, visiting nurses, and other community programs helping individuals in need or individuals in isolation or quarantine or both.

 

Social Support Coordinators shall receive referrals for social services from the contact tracing and case investigation processes.

 

Social Support Coordinators shall assist with connecting individuals to resources they need.

 

Social Support Coordinators shall enter information into the appropriate systems with completeness and accuracy at the time of the interview. Social Support Coordinators shall not create separate files or databases outside of the appropriate IT systems as approved by the Department.

 

Social Support Coordinators shall maintain daily contact with the Social Support Coordinator Supervisor.

 

Social Support Coordinators shall establish and maintain contact with other Social Support Coordinators to maintain statewide coverage and awareness of available resources

Current Pennsylvania Resident Required

Qualifications

Minimum Qualifications:

  • Candidates preferred to have experience working with community resources and social services 
  • Ability to speak, read, and write English; fluency in languages other than English is advantageous
  • Ability to exhibit a professional, positive attitude and work ethic
  • Ability to show empathy and be nonjudgmental towards distressed individuals Excellent interpersonal skills and ability to interact professionally with people from
  • diverse cultural, racial, ethnic, gender, and socioeconomic backgrounds during a time of crisis and distress
  • Have a demonstrated commitment to supporting communities who have experienced systemic oppression and bias (e.g. people of color, LGBTQ+ people, immigrants, justice involved persons, etc.)
  • Excellent organizational and communication skills Critical thinking and sound judgment required
  • Ability to handle confidential information with discretion and professionalism Proficiency in basic computing and ability to access and navigate various databases and software programs following training
  • Must have access to good/reliable WiFi or internet

Education/Experience:

  • Bachelor’s degree
  • Experience in social work, mental health counseling
